The influenza of that season, however, was far more than a cold. In the two years that this scourge ravaged the earth, a fifth of the world's population was infected. The flu was most deadly for people ages 20 to 40. The pattern of morbidity was unusual for influenza, which is usually a killer of the elderly and young children. It infected 28% of all Americans. An estimated 675,000 Americans died of influenza during the pandemic, ten times as many as in the world war. Of the U.S. soldiers who died in Europe, half of them fell to the influenza virus and not to the enemy. An estimated 43,000 U.S. servicemen mobilized for WWI died of influenza. 1918 would go down as unforgettable year of suffering and death and yet of peace.

The Possibility Of An Influenza Or Bird Flu Pandemic Today
Concern is growing worldwide about a virulent strain of avian influenza - or bird flu - that is spreading through countries in East Asia.  In patients infected with the H5N1 virus, clinical deterioration is rapid. In Thailand, the time between onset of illness to the development of acute respiratory distress was around six days. In Turkey, clinicians have observed respiratory failure three to five days after symptom onset. Another common feature is multiorgan dysfunction. Common laboratory abnormalities, include leukopenia (mainly lymphopenia), mild-to-moderate thrombocytopenia, elevated aminotransferases, and with some instances of disseminated intravascular coagulation.
Limited evidence suggests that some antiviral drugs, notably oseltamivir - commercially known as Tamiflu - can reduce the duration of viral replication and improve prospects of survival, provided they are administered within 48 hours following symptom onset. However, prior to the outbreak in Turkey, most patients have been detected and treated late in the course of illness. Moreover, oseltamivir and other antiviral drugs were developed for the treatment and prophylaxis of seasonal influenza, which is a less severe disease associated with less prolonged viral replication. Recommendations on the optimum dose and duration of treatment for H5N1 avian influenza, also in children, need to undergo urgent review, and this is being undertaken by WHO.
Peter Cordingley, spokesman for the World Health Organization regional office in Manilla told the AFP news agency: "If the H5N1 virus attaches itself to the common human flu virus and if it is then effectively transmitted, it has the potential to cause widespread damage. The common human flu virus is far more infectious than the Sars virus and can spread by aerosol and not just through droplets as in the case of Sars virus. The mortality rate is far higher than that of the Sars virus."

Avian influenza is a disease of birds caused by influenza viruses closely related to human influenza viruses. Transmission to humans in close contact with poultry or other birds occurs rarely and only with some strains of avian influenza. The potential for transformation of avian influenza into a form that both causes severe disease in humans and spreads easily from person to person is a great concern for world health.

It is also not known if Tamiflu will be effective for treating flu in people with chronic heart disease or respiratory disease, or in people with medical conditions that are sufficiently severe or unstable to potentially require admission to hospital.

UN officials warned that the Avian Bird Flu outbreak posed the "gravest possible danger" of becoming a global pandemic; and the World Health Organisation has warned governments they must stockpile vaccines in preparation for a pandemic. Tamiflu has been selected by the World Health Organisation as its drug of choice to protect against avian bird flu and for use in case of widespread human outbreak.
